Our preferred method for accepting referral is through Medical Objects.

For Patients

Making appointment

A referral letter from your GP or a medical specialist is required. The referral letter can be sent to us by your referring doctor via Medical Objects or if you have a copy, you can email or fax it to us. Our doctors will triage the referral, and we will then contact you to set up an appointment. If you have specific requirements for the day and time of the appointment, it is best to call us directly.

A GP referral is valid for 12 months and a specialist referral is valid for 3 months. A valid referral letter allows you to claim your Medicare rebate.

Our usual wait time is approximately 4-6 weeks. However, if you have no preferences on which rheumatologist to see, we usually are able to offer you an appointment within 2 weeks.

Please call for information about the current wait time and fee for our rheumatologists.

For patient who has seen our rheumatologists previously but has not been seen in the past 12 months, you will be seen as new patient.

Paperwork to bring when attending First Appointment
  1. Your GP / Specialist referral letter, if this has not been sent to us prior to your appointment.
  2. Your Medicare Card, Pensioner Concession Card, Health Care Card or DVA Card (if you have one). We offer concession to pensioner and healthcare card holder. Please present your concession card to the front desk.
  3. X-ray/ Ultrasound/CT scan / MRI results and pathology results such as blood and urine test results and previous documentations related to your consultation.In most cases, your referring doctor will attach all or some of these results in the referral letter. However, there are circumstances when this is not the case. As most of these results are available electronically from the service providers, please inform us where you have had your tests done to allow us sufficient time to make contact and download the results prior to your appointment.
  4. If you are a new patient, you will need to complete an Initial Visit Form. You can download the Initial Visit Form here , complete the form and email back to us prior to your consultation.If you choose to complete the Initial Visit Form in the waiting room, please arrive 10 -15 minutes earlier than your scheduled appointment time. Allowing sufficient time for the paperwork to be completed will reduce chances of delay to your appointment and impacting patients’ schedule.
  5. A list of your current medications and dosage you are taking, any allergies or side – effects you may have with previous medications.
First Consultation

Allow 30 – 45 mins for your first consultation. Our rheumatologist will assess your symptoms and conditions, which will include your / your family medical history. Additional investigations such as blood test or imaging maybe ordered if existing reports are insufficient to support a diagnosis. Our rheumatologist will work out a management plan with you, and may prescribe treatment such as medication, physiotherapy, hydrotherapy or joint injection. Follow up review will be scheduled if required.

Reminder/ Rescheduling / Cancelation

You will receive a text reminder for your appointment one week before your scheduled appointment. Please reply to confirm. If you would like to reschedule, kindly give us a call or send us an email. We kindly ask that you give us at least 3 days notice for any cancellation.

For booking that we do not receive a reply after 3 attempts of contacting you, your appointment will unfortunately be cancelled. So please ensure you provide us with your best contact number and email address.

To ensure fair access to health care for other patients, new patient who has cancelled at last minute /failed to turn up without reasonable ground may not be offered a second appointment in this practice.

Fee for Consultation

We are a private billing medical practice. Full fee payment is required after the consultation. We do not offer bulk-bill consultation. An initial consultation fee is applicable for your first appointment, and a review fee is applicable for subsequent review appointment. Higher fee is applicable for a complex case consultation.

Medicare rebate is available to partly cover the consultation fee, with the exception for joint aspiration/injection. Medicare rebate can be processed electronically on the day for you if you have registered with Medicare online ( i.e. provided Medicare your bank account). Please contact the practice for the current fees schedule / Medicare rebate amount.

We offer fee concession for Pensioner Concession Card and Health Care Card holders.

No gap fee for Department of Veterans’ Affairs Gold Card holder and Australian Defence Force personnel covered by BUPA.

Telehealth / Phone appointment

Telehealth (online video call) appointment is available on request for review patient only. There are circumstances when a Telehealth appointment is not recommended due to the complexity of patient’s conditions, or specific PBS rules. Please check with the practice if you would like to request a Telehealth review appointment.

Review fee applies and Medicare rebate is available for a Telehealth review appointment.

Review fee also applies for phone review appointment, however, Medicare rebate available for a phone appointment is lower than a face-to-face appointment.

Dr. Chin Ng consults at Sunnybank on Monday, Wednesday, Thursday and Friday, at Cleveland on Tuesday and one Friday a month.

Dr. Ravini de Silva consults at Sunnybank on Monday and Tuesday.
